Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation # - 70Z02326QPPC00001. This solicitation is issued as a Request for Quotation (RFQ). A statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99. The North American Industry Classification System Code (NAICS) is 713940 and the business size standard $17.5 million. This is a total small business set aside. Description of requirements for the items to be acquired- Provide 160 gym/fitness memberships to all vendor locations in the Topeka and surrounding area. Provide list of gyms and their locations. Must have multiple locations in the Topeka area, ie. At least two. Facilities must provide cardio (i.e., treadmill, elliptical, rowing, etc.) and weightlifting equipment (i.e., free weights, dumbbells, weight machines, strength machines, resistance machines, etc). Provide a quarterly report detailing gym membership usage. This report shall include, at a minimum, the total number of visits, the number of members who visited the gym, and the last usage date per member. The report shall be submitted to the Contracting Officers Representative (COR) within five (5) business days of the end of each calendar quarter. Delivery Date � See period of performance.
