Description

The Contractor shall provide all labor, supplies, materials, and equipment to remove existing monument information plaque, restore information plaque, excavate area, form, pour and install new concrete pad, form and pour in place new monument base and re-install restored information plaque. The contractor shall excavate designated area as per Attachment 2. All excavated material shall be set aside for finish grading after pad and monument are complete. The area of excavation for the 20 x 20 pad as per Attachment 3 be deep enough to allow for a 6-inch thick 20 ft by 20 ft concrete pad to have a minimum of 5 inches of concrete below the ground level and 1 inch extending beyond ground level. The contractor shall place 3 inches of 304 aggregate under the 6-inch-deep pad prior to the pour of pad. The concrete pad shall have a 1 inch to 10 ft slope from side to side as indicated on Attachment 4. The contractor shall install 4x4 wire mesh and tie, for the entire formed pad area except in the location of proposed monument. The contractor shall construct a monument and base as per Attachment 6. The dimensions of the monument shall be field verified and relative to the Government Furnished Plaque. The contractor shall remove the existing bronze plaque from the concrete monument, thoroughly clean off all oxidation and corrosion as well as all paint on lettered portion or any other portion of plaque that is painted.
