Description

Amendment 0001 is hereby issued to add to: 1)H.16 - Quality Assurance Plan (QAP), 2) Attachment 11- Quality Assurance Plan 3) 52.222 - 90 - Addressing DEI Discrimination by Federal Contractors (APR 2026). The attached questions and answers regarding the requirment are also added. All other Terms, Conditions, and Line Items remain the same. THIS AMENDMENT EXTENDS THE SOLICITATION CLOSING DATE/TIME TO: 06/03/2026 - 14:00 America/New_York ------------------------------------------------------------------------------------------------------------------------------------------------------------------------------- This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with part 12. This announcement constitutes the only solicitation. Offers are being requested and a separate written solicitation will not be issued. Solicitation number 15BFA026R00000003 is issued as a Request for proposal (RFP) for Comprehensive Medical Services. This acquisition is not set-aside for small business concerns. This solicitation incorporates provisions and clauses by reference. The full text of provisions and clauses may be accessed electronically at www.acquisition.gov. (ii) Please see Section B in the attached solicitation for details regarding Line-Items. (iii) No brand name requirements for the items are to be acquired. (iv) Services rendered will be provided at FMC Fort Worth located in Fort Worth, Texas. The date and times will be determined after award. Please see attached solicitation for details. (v) Please see Sections K, L & M in the attached solicitation documents for a list of contract clauses that apply to this acquisition. (vi) Please see Sections G, H & I in the attached solicitation documents for a list of contract clauses that apply to this acquisition. (vii) Offers shall be received via email (kmfisher@bop.gov and BOP-ADM-FAO-CMS-S@bop.gov) on or before the deadline as listed on the SF-33 Block 9.
