Description
Amendment 001 to Notice W912LR26QA021 The purpose of this amendment is to provide answers to requests for information and update the solicitation in accordance to responses from the Government. The following updates and changes have been included: (a) Answers to Requests for Information. (b) The Performance Work Statement has been updated in accordance to responses by the Government (c) Clauses 52.217-8 and 52.217-9 has been updated to state period as "calendar" days. (e) Provision 52.212-1 addendum, Instructions to Quoters, has been updated in accordance to Question #2 of Requests for Information. (f) PWS has been replaced with Revision 1, dated 28 May 2026, with highlighted changes. (g) Consolidated Requests for Information and responses have been included as an attachment. There are no further changes to terms, conditions, performance standards or requirements other than those expressly identified in this amendment. This announcement constitutes the only solicitation. Quotations are being requested, and a separate written solicitation will not be issued. The Government intends to issue a Request for Quotation for the purchase of commercial services for Occupational Health Medical Surveillance Examination. This acquisition is set aside for small business concerns. This solicitation incorporates provisions and clauses by reference. The full text of provisions and clauses may be accessed electronically at www.acquisition.gov. This acquisition is not subject to the Defense Priorities and Allocations System (DPAS). NAICS Codes: 621111 Offices of Physicians (except Mental Health Specialists) Size Standard: $16 million in average annual receipts PSC: Q403 Period of Performance for Base Period: 15 June 2026 to 30 April 2027 Place of Performance: Bldg 589, Camp Santiago Joint Training Center, Salinas, PR. Delivery and Acceptance FOB Point: DESTINATION Submission Instructions Deadline: 03 JUNE 2026 at 11:00 AM AST Submission Method: Quotes shall be submitted electronically to ng.prarng.purchasing-and-contracting.mbx@army.mil. Include the solicitation number in the subject line. Questions: Submit questions to the same email address no later than 27 MAY 2026. Questions must relate to clarification of requirements, terms, and conditions only. The contracting office will not address questions explicitly stated in this solicitation and its attachments. Basis of Award The Government will issue a Firm-Fixed Price (FFP) Purchase Order to the lowest quote that conforms to the material aspects of the solicitation in accordance with solicitation instructions (see 52.212-1) and evaluation (52.212-2).
