Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in RFO Part 12, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Defense Health Agency Contracting Activity, Healthcare Contracting Division � West, Foxtrot Branch, Bldg 339 146 Clark Road, Ft Shafter, HI 96858, intends to solicit, negotiate, if necessary, and award firm fixed-price contract for a water purification system, reverse osmosis, hemodialysis for the Brian D. Allgood Army Community Hospital, Korea. This is a request for quotation (RFQ). The solicitation number is HT941026QE020. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2026-01. The Standard Industrial Classification Code is 4610, size standard is 1000 employees and the NAICS code of 333310. NOTICE TO QUOTERS: All sources may submit a quote as applicable. To comply with the US-ROK Status of Forces Agreement (SOFA) and USFK Regulation 715-2 (Local Sourcing Preference), the Government will evaluate quotes using a Tiered Evaluation process. Offerors must prominently state on the cover page of their quote whether they are submitting as a Tier 1 (Korean Local Source) or a Tier 2 (U.S./Off-Shore Source). Failure to clearly identify the appropriate Tier may result in the quote being deemed non-responsive. Provision at 52.212-1, Instructions to Offerors -- Commercial, and its addendum applies to this acquisition and a statement regarding any addenda to the provision. Provision at 52.212-2 Evaluation - Commercial Items and its addendum applies to this acquisition. Offerors shall include a completed copy any provisions specified in the combined synopsis/solicitation attachment. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, and its addendum applies to this acquisition. All applicable provisions and clauses are in the combined synopsis/solicitation attachment. The Defense Priorities and Allocations System (DPAS) and assigned rating is not applicable. The Government intends to award a firm fixed-price contract to the successful quoter. Contract Requirements - The successful quoter shall be required to meet the requirements of the combined synopsis/solicitation attachment.
