Description
The Department of Veterans Affairs is conducting Market Research for the requirement of Ambulance Service under NIACS code 621910 and Product Service Code V225. If you are interested in submitting a quote after the RFQ has been published to SAM.gov please complete the following. Failure to provide any of the following will result in your response being rejected from data composed for our market research. Prime Vendor s Name Prime Vendor s UEI Prime Vendor s Cage code ACTIVE Social economic set-asides Does the Prime Vendor have an EMFTS license. Will a Subcontractor be used in the requirement. Note: A EMFTS license is a requirement to operate in the state of Ohio. We are a federal agency and we work on federal land but provide services to/from other clinics and medical centers on public and private property. We have had a number of protest in the past over the requirement of the EMFTS license and everyone has been decided in the Governments Favor.
