Description
General Information (REQUEST FOR INFORMATION/ SOURCES SOUGHT ONLY) 1. PROGRAM OBJECTIVE: The G.V. Sonny Montgomery VA Medical Center requires the following services to be provided in a private hospital, office, or clinic environment to Veterans, primarily residing in but not limited to the counties of Lowndes, Monroe, Oktibbeha, Clay, and Noxubee County, MS. 2. SCOPE: This site is classified as a Primary Care Community Based Outpatient Clinic CBOC offers both medical and mental health care and may offer support services such as pharmacy, laboratory, and x-ray. Primary Care CBOCs are required to provide both primary care and mental health services. **Mental Health services will be provided onsite by VA staff, and or by VA staff via Telehealth. 3. Place of Performance: Services shall be performed by contractor within the city limits of Columbus, Lowndes County, MS on behalf of G.V. Sonny Montgomery VA Medical Center at 1500 East Woodrow Wilson Ave, MS 39216 4. PERIOD OF PERFORMANCE: 5-year (Base + 4, 1-year options) Tentative first day of service: April 1, 2027 Note: The NAICS Code for this procurement is 621498; size standard $8 million. The Service Contract Labor Standards will also apply to this procurement. This Sources Sought Notice is for market research purposes only and shall not be considered as a request for quote or an obligation on the part of the Government to acquire any services. No entitlement to payment of direct or indirect costs or charges by the Government will arise as a result of the contractor s submission of this Sources Sought Notice or the Government s use of such information. The Government reserves the right to reject in whole, or in part, any contractor s input resulting from this Sources Sought Notice. No contractual arrangement will be made as a result of this notice. Information submitted in response to this Sources Sought Notice will not be returned. All interested parties that are capable of providing the required services shall submit a response via email only to karen.battie@va.gov no later than June 19, 2026, at 12:00 PM CST. The responses shall be limited to 7 pages and must include all the information below. a. Contractor names, SAM Unique Entity Identifier (UEI) number, address, point of contact, title, phone number, and email address b. Brief description of the company background, capability to meet this requirement (may include nurse resumes, facility/company accreditations, type of equipment used, etc.), and past or current performance under contract (civilian or government). c. Contractor s Business Size and socioeconomic status, including Veteran Small Business Certification (VetCert), if applicable. d. Contractor s interest in responding to a solicitation if issued.
