Page 2 of 2 The Northport VA Medical Center (VAMC) within the New York/New Jersey VA Health Care Network (VISN 2) has a need for residential housing services at the VA Medical Center as part of the Health Care for Homeless Veterans (HCHV) program. The goal of the HCHV program is to remove homeless veterans from the street or other places considered unfit for human habitation and place them in community-based, residential environments with sufficient on-site psychosocial support services to meet the needs of those veterans and place them in permanent housing as rapidly as possible. This program will be located in a designated building and floor, on-site at the Northport VAMC at 79 Middleville Road, Northport, NY 11768. Request for Proposal (RFP) 36C24226R0065 will be available for download from this website in approximately 15 days. No questions should be submitted at this time. No hardcopies will be sent. Once the solicitation is posted, questions must be submitted in writing via email to the Contract Specialist at Veronica.Baldwin1@va.gov. Questions must be received a minimum of ten (15) business days before the closing date listed in the RFP. If warranted, responses to inquiries will be incorporated into a written amendment posted to the SAM.gov at https://sam.gov/. Verbal questions and phone calls will not be addressed. Offerors are encouraged and are responsible for routinely checking this website for any solicitation amendments that may be issued throughout the proposal period. This requirement is being solicited as unrestricted. The NAICS Code for this requirement is 624229 Other Community Housing Services, Small Business Size Standard $19.0 Million. Contractors are required to be registered with the System for Award Management (SAM) at https://sam.gov/ prior to contract award.
