Description
Title: Controlled Substance Destruction VISN 21, Station 593, Las Vegas 1. INTRODUCTION The Department of Veterans Affairs (VA), Network Contracting Office 21 (NCO 21), is issuing this Sources Sought notice as a means of conducting market research to identify parties having an interest in, and the resources to support, a requirement for Controlled Substance Destruction. The results of this market research will contribute to determining the method of procurement and identifying potential Service-Disabled Veteran-Owned Small Businesses (SDVOSBs) or Veteran-Owned Small Businesses (VOSBs). The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 541380 (Controlled Substance Destruction.) The Product Service Code (PSC) is Q399 (Medical � Other). 2. DISCLAIMER THIS SOURCES SOUGHT IS FOR INFORMATIONAL PURPOSES ONLY. THIS IS NOT A REQUEST FOR PROPOSAL (RFP) OR QUOTE (RFQ). It does not constitute a solicitation and shall not be construed as a commitment by the Government. Responses in any form are not offers and the Government is under no obligation to award a contract as a result of this announcement. No funds are available to pay for preparation of responses to this announcement. 3. SCOPE OF REQUIREMENT / CAPABILITY REQUIREMENTS The contractor shall provide all personnel, equipment, supplies, tools, materials, supervision, and other items necessary to perform Incubation/Speciation of Viable Samples for � Media Fill, Gloved Fingertip and Thumb Sampling, Viable Surface. Performance will occur for Station 593 at the North Las Vegas VA Medical Center, 6900 North Pecos Road, North Las Vegas, NV 89086, and its associated regional clinics under the VA Southern Nevada Healthcare System. Key technical capability requirements include: Provide routine software updates, firmware upgrades, and hardware upkeep for passive RTLS sensors, tags, and tracking systems. Ensure personnel hold valid manufacturer certifications required to work on specialized medical and enterprise logistics tracking components. Adhere strictly to VA information security, network connectivity, and patient safety privacy mandates Area Las Vegas - U.S. Department of Veterans Affairs. 4. CAPABILITY STATEMENT SUBMISSION REQUIREMENTS Interested firms are requested to submit a capability statement that directly addresses their ability to fulfill this requirement. Submissions must not exceed 5 pages and must include: Company Information: Company Name, Address, Unique Entity ID (UEI) from SAM.gov, point of contact name, phone number, and email address. Socio-Economic Status: Explicitly state if your firm is a certified SDVOSB, VOSB, Small Business, or other classification. Vendors must be verified and visible in the Small Business Administration (SBA) Veteran Small Business Certification (VetCert) portal to qualify for VA veteran set-asides. Technical Capabilities: A summary of technical capabilities and experiences demonstrating the ability to perform the scope outlined above. Past Performance: Provide a brief summary of at least two (2) similar logistics or tracking system maintenance contracts completed within the past three (3) years, including contract numbers, dollar values, and points of contact. 5.
