THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. This is not a solicitation for proposals, proposal abstracts, bids, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform Catholic Pries Services at the William S. Middleton Memorial VA Hospital located in Madison, WI. Responses to the information requested will assist the Government in determining the appropriate acquisition method. The Great Lakes Acquisition Center (GLAC) Milwaukee, WI is seeking contractors to provide to William S. Middleton Memorial VA Hospital 2500 Overlook Terrace, Madison, WI 53705 for Roman Catholic Priest Services. Roman Catholic Priest Duties will include but not limited to the following: A. Conduct Eucharistic Celebrations to include weekly Mass and other holiday services at his discretion. Provides religious services held in the Madison VA Chapel. B. Contractor will work 10 hours a week (9 hours providing for the sacraments to our Roman Catholic Veterans, attending Chaplain Service meetings/functions and continued training in TMS as well as responding to emails. C. Respond to patient and staff requests for assistance promptly & in a friendly & cooperative manner. D. Listen to feedback from patients/staff, positive and negative, acts to resolve complaints within his control, and reports feedback to Chief, Chaplain Service in a timely manner. E. Provide verbal information that is thorough and correct, using clear and precise communication methods to ensure that others understand what is being shared. Contractor shall provide any required information in a timely manner. F. Consistently has open dialogue with Chief, Chaplain Service, other Chaplains, and Chaplain Secretary as it relates to Chaplain Service. G. Cooperates with other Chaplains to ensure that Roman Catholic veterans receive proper care from the Chaplain Service. H. Contractor will attend weekly Chaplain Service meetings in-person. I. Roman Catholic Priest interacts with Hospital staff and is expected to share relevant, non-confidential information with other disciplines. Non-confidential issues that pertain to the Veteran s spiritual health care planning must be shared with fellow providers/Chaplains to ensure continuity of care. J. All Chaplain Service contracts must be included in workload report and given to the Chaplain Service office. K. Arranges for the atmosphere and symbolism of worship. Provides religious instruction, such as individual or group discussions, and distribution of appropriate literature to patients as required. L. Chapel Offices are provided for the Roman Catholic Priest to use in performance of duties. M. Chapel is provided for performance of weekly Mass services. N. Contractor must be willing to respond to on-call emergencies except on designated days off. O. The Contracted Roman Catholic Priest will in advance, prepare a monthly routine schedule for services provided under this contract, based on the number of visits the contractor will conduct per month, with services generally being provided on the same days each week of the month. Report must be presented to Chief, Chaplain Services for approval prior to beginning of the month. SPECIAL CONTRACT REQUIREMENTS Contractor must be an Ordained Roman Catholic Priest affiliated to the Archdiocese for the Military Services, USA. This affiliation certifies that the applicant is in good standing and qualified to perform the ministry required in a VA setting. This is a necessary requirement to ensure spiritual services are provided by a recognized and authorized individual. a. The Ordained Roman Catholic Priest shall have the training of their religious faith group that is in keeping with their established standards. b. Personnel assigned by the contractor to perform the services covered by this contract shall be proficient in written and spoken English (38 USC 7402). c. Contractor s assigned personnel must be computer literate as patient documentation is directly is entered in the patient s electronic medical record. The following NAICS Code and Small Business Size Standard has been identified for this requirement: 813110 (All information shall be submitted in Word or PDF format, not to exceed 25 pages, including all attachments.) The Government will evaluate market information to ascertain potential market capacity to provide services consistent in scope and scale with those described in this notice and otherwise anticipated.
