The RFQ and associated documents (listed below) are attached: 0. RFQ and submission instructions 1. Sample Subcontract 2. Statement of Work 3. Past Performance Reference Information Form (submit with Quote regardless of value) 4. Rep Cert Form (required with submission if total Quote exceeds $150K) *With your Quote submission, you must certify in writing that you meet and agree to all of the Mandatory Requirements outlined within Doc 0.
