Description
Attachment with QNA updated 2:19 PM PDT. Amendment 0001 is issued for RFQ-26-PHX-035, HHCC Clinical Staffing Services, to provide the Government�s responses to vendor questions and to extend the quote due date. The quote due date is extended from June 29, 2026 at 9:00 AM PDT to July 7, 2026 at 9:00 AM PDT. Offerors shall review Amendment 0001 and the attached Vendor Questions and Answers in full. Amendment 0001 is a required supplement to each offer. Offerors must acknowledge Amendment 0001 with their quote submission in accordance with the RFQ instructions. All other solicitation terms and conditions remain unchanged unless expressly modified by Amendment 0001. The Indian Health Service, Phoenix Area Office, is issuing this combined synopsis/solicitation for non-personal clinical staffing services for Hopi Health Care Center (HHCC) in Polacca, Arizona. The Government intends to award multiple fixed-price indefinite-delivery/indefinite-quantity (IDIQ) contracts under the Buy Indian Act / HHSAR Subpart 326.6 Indian Small Business Economic Enterprise (ISBEE) set-aside. This solicitation is for clinical staffing support in the following anticipated staffing categories: Emergency Department Physician Services; Inpatient/Hospitalist Physician Services; Emergency Department Registered Nurse Services; Inpatient Registered Nurse Services; and Outpatient Registered Nurse Services. Offerors may propose for one, several, or all staffing categories. The resulting IDIQ contracts will establish awarded staffing categories, fixed fully burdened hourly rates, ordering procedures, and contract administration requirements. Award of a parent IDIQ contract does not guarantee task orders, hours, funding, candidate placement, staffing category utilization, or any share of future orders beyond the stated minimum guarantee. All solicitation requirements, instructions, evaluation factors, pricing requirements, performance requirements, forms, and contract administration procedures are contained in the RFQ and attachments. Offerors shall review the RFQ and all attachments in full before submitting a quote. The attachments include the rate schedule, Business Associate Agreement, tax exemption letter, Performance Work Statements with embedded QASP tables, Indian Economic Enterprise representation form, Past Performance Questionnaire, ordering procedures, and applicable contract clause matrix. Quotes shall be submitted electronically in accordance with the RFQ instructions by the date and time stated in the solicitation. Questions shall be submitted in writing by the deadline stated in the RFQ. Responses to questions, if provided, will be posted through SAM.gov by amendment or written notice. SAM.GOV FOLLOW REQUIREMENT DISCLOSURE To receive up-to-date information on this requirement, including answers to vendor questions, modifications to the requirement, attachment updates, or extensions to the offer due date, interested vendors should use the �Follow� feature in SAM.gov to receive system notifications. The Government is not responsible for an interested vendor�s failure to monitor SAM.gov for amendments, answers to questions, attachment updates, or other notices.
