Point of Contact for this Sources Sought is: veronica.flores@va.gov Department of Veterans Affairs Network Contracting Office -15 3450 S. 4th Street Trafficway Leavenworth, KS. 66048� STATEMENT OF REQUIREMENT (SOR) Title:� Surgical Laser Services Kansas City VA Medical Center Period of Performance:� 5 Year Ordering Period Location:� Kansas City VA Medical Center (KCVAMC), 4801 East Linwood Boulevard, Kansas City, MO 64128-2226 1. Purpose The Kansas City VA Medical Center (KCVAMC) Surgical Services requires comprehensive surgical laser support to facilitate complex surgical procedures. Surgical lasers are essential for precision ablation and are used across a variety of clinical applications. This requirement includes the provision of laser equipment, accessories, and qualified technician services on an as-needed basis. 2. Scope of Work The contractor shall provide the following equipment and services: 2.1 Equipment Types 120H Holmium Laser w/Moses Technology laser system must be housed at the VA Facility throughout the duration of the contract. CO laser systems as required CO Direct Line of Sight laser systems as required GreenLight laser systems as required KTP laser systems as required Lithotripsy laser systems as required If additional surgical lasers will be required there will be a modification to add the additional lasers. 2.2 Accessories and Supplies Laser fibers Handpieces Instruments Other necessary accessories 2.3 Services Qualified technician support for equipment setup, operation, and malfunction response Delivery, setup, and removal of equipment per procedure schedule Maintenance documentation for all equipment used 3. Service Delivery Requirements Services shall be provided� on a case-by-case basis� in accordance with the contract. All equipment must be in� optimum operating condition� at the time of use. Only the� OR Scheduler or authorized designee� may request laser services. Services will be requested� only during normal business hours, Monday through Friday, 7:30 a.m. to 7:00 p.m. No services� will be requested or reimbursed outside these hours, including weekends and federal holidays. 4. Contractor Responsibilities 4.1 Arrival and Check-In Contractor must check in with the� OR Unit Manager or designee� upon arrival (5th Floor, OR). Equipment must be delivered to the� OR front desk at least 1 hour prior� to the scheduled procedure. 4.2 Equipment Pre-Testing If the laser is� not housed at KCVAMC, it must undergo� pre-testing� by Healthcare Technology Management. If the laser is� already housed at KCVAMC, the contractor must still arrive 1 hour prior with all required supplies. 4.3 Failed Equipment If contractor-owned equipment� fails inspection or pre-test, the VA is� not obligated to reimburse� for the cancelled or rescheduled procedure. 4.4 Sterilization Deliver such items to� Sterile Processing Service (SPS)� (Ground Floor)� at least 2 business days prior� to the procedure. A count sheet and IFU (Instructions for use) is to be made available. 4.5 Technician Support A qualified laser technician/operator must: Arrive with all requested equipment, accessories, and fibers. Provide a� hard copy of current maintenance records� for all equipment used. 4.6 Equipment Handling Contractor is responsible for: Timely� drop-off and setup� of equipment prior to treatment. Removal� of all equipment after the procedure. 5. Delivery Requirements Delivery Location: Department of Veterans Affairs Kansas City VA Medical Center 4801 East Linwood Boulevard Kansas City, MO 64128-2226 Federal Holidays Observed by the VA Medical Center New Year's Day Labor Day Martin Luther King, Jr. Day Columbus Day President's Day Veterans Day Memorial Day Thanksgiving Day Juneteenth Christmas Day Independence Day and any other day specifically designated by the President of the United States.
