Description
The purpose of this amendment is:- To replace pages 12, 13, and 14 of the solicitation (Section B PART II: PRICE - GROUP LIFE INSURANCE) with pages 2, 3, and 5 of this amendment To replace pages 18 - 25 of the solicitation (Section C.1 HEALTH INSURANCE SERVICES) with pages 6-13 of this amendment
