Description
The G.V. Sonny Montgomery VA Medical Center requires Primary Care Community Based Outpatient Clinic (CBOC), that offers medical and mental health care services to Veterans to be provided in a private hospital, office, or clinic environment to Veterans, primarily residing in but not limited to, the counties of Adams, Jefferson, Wilkinson, and Franklin County, MS. NAICS CODE: 621498, SIZE STANDARD: $25.5 MILLION All interested parties that are capable of providing the required services shall submit a response via email only to Tiffany.Lee-Franklin@va.gov no later than December 8, 2025, at 4:00 PM CST. The responses shall be limited to 7 pages and must include all the information below: a. Contractor names, SAM Unique Entity Identifier (UEI) number, address, point of contact, title, phone number, and email address b. Brief description of the company background, capability to meet this requirement (may include nurse resumes, facility/company accreditations, type of equipment used, etc.), and past or current performance under contract (civilian or government). c. Contractor�s Business Size and socioeconomic status, including Veteran Small Business Certification (VetCert), if applicable. d. Contractor�s interest in responding to a solicitation if issued.
