Description
Biosense Webster CARTO� 3 and SMARTABLATE� System Maintenance SCOPE: This Purchase Order shall be for a one (1) base year. The Biosense Webster System Maintenance Agreement Purchase Order shall provide preventive maintenance service on the CARTO� 3 and SMARTABLATE� systems (the �Covered Equipment�) at HARRY S TRUMAN MEMORIAL VA HOSPITAL. Additionally, it provides corrective maintenance on the system if it shall be malfunctioning or inoperable at a mutually agreeable time and loaner equipment, when deemed necessary by the Contractor. Training materials shall be provided for base software upgrades that affect user operation of the base software. Equipment to be covered by this agreement: CARTO� 3 System Serial Nbr: 13446 SMARTABLATE� System Serial Nbr: G4C-2022 SMARTABLATE� Irrigation Pump Serial Nbr: G4CP-1924 SMARTABLATE� Remote Control Serial Nbr: G4RC-1894 PURCHASE ORDER PERIOD: Base Year Period of Performance from 12/01/2025 � 11/30/2026
