Description
1.0 DISCLAIMER This Request for Information (RFI) is issued in accordance with the FAR 10.002(b)(2) to determine the availability and technical capabilities of qualified sources. Please respond to the questions in section 3.0. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112, Surgical and Medical Instrument Manufacturing, with a small business size standard of 1,000 employees. The FAR Provision found at 52.204-7(b)(1) requires that All interested Offerors must be registered in SAM prior to submitting an offer or quotation. You may access the SAM website at https://www.sam.gov/portal/public/SAM/. All quoters must be certified with the appropriate NAICS code on the SAM website. All SDVOSB/VOSB offerors must be verified by the Small Business Administration at the time of quote submission. This notice is for planning purposes only and does not constitute a commitment on the part of the Government to award a contract, nor does the Government intend to pay for any information submitted as a result of this notice. This RFI is part of a continuous process for obtaining the latest information on the commercial status of the industry with respect to their current and near-term abilities. The information provided herein is subject to change and in no way binds the Government to solicit for, or award, a competitive contract. It is emphasized that this is a notice solely for planning and informational purposes. 2.0 DESCRIPTION OF ANTICIPATED NEED The Buffalo VA Medical Center is working to procure retinal probes that are compatible with our new Cryomatic MKII. The Buffalo VAMC is seeking to purchase four (4) Cryosurgical probes, two standard retinal probes and two mid-reach retinal probes. Procuring this equipment will ensure device uptime in our OR so that Veterans are receiving the best care possible. The probes must have the salient characteristics listed below: SALIENT CHARACTERISTICS OF BULK ROCK SALT The probes must be compatible with the Keeler Cryomatic MKII Cryosurgical unit. The probes must be compatible with CO2 or N2O. The probes must be autoclavable. The probes must have a tip diameter less than 12mm. The probes must have a total length of less than 5m. The probes must include a standard warranty. DELIVERY REQUIREMENTS All items must be delivered to the Buffalo VAMC located at 3495 Bailey Avenue, Buffalo, NY 14215 within 30 days from date of award. The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory.
