Description
Amendment 0001 This solicitation is amended with the following changes: The offers due date is extended to January 22nd, 2026, at 5:00 PM CT. The questions due date is extended to January 15, 2026, at 12:00 PM CT. The Period of Performance (PoP) and ordering period dates are revised in Attachment 1 - Solicitation Terms and Conditions. The requirement to submit a quality assurance plan for the technical acceptability evaluation was removed in Attachment 1 - Solicitation Terms and Conditions. The requirements for Optometrist Services were removed in Attachment 2 - Statement of Work (SOW). This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Part 12. This announcement constitutes the only solicitation; offers are being requested, and a separate written solicitation will not be issued. Solicitation number 12639526Q0013 is issued as a Request for Quotation (RFQ) for professional services providing occupational prescription safety eyewear supplies and pertinent accessories. This acquisition is set-aside for small business concerns. This solicitation incorporates provisions and clauses by reference. The full text of provisions and clauses may be accessed electronically at www.acquisition.gov. The applicable North American Industry Classification Standard Code is 339115 Ophthalmic Goods Manufacturing. The small business size standard is 1,000 employees. This acquisition is a Total Small Business Set-Aside. All responsible sources may submit a quotation which will be considered by the agency. See �Attachment 1 - Solicitation Terms and Conditions� for applicable clauses, instructions, and evaluation criteria.
