Description

Combined Synopsis/Solicitation # HT940626QE019 is issued as a Request for Quote (RFQ). The combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Regulation as indicated in the attachment. SF Form 1449 is being utilized to generate the applicable clauses. The Defense Health Agency, Healthcare Contracting Division- Southeast intends to procure a firm-fixed price contract for personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to provide a middleware solution for Point of Care Testing (POCT) connectivity that can transmit patient test results into the hospital�s current laboratory information system for Roche Accu-Chek Inform II and Abbbott i-STAT 1 located at Naval Medical Center Camp Lejeune (NMCCL), NC. This requirement is being solicited in accordance with Federal Acquisition Regulation FAR Part 12.6; Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services. Competition Approach: Full and open competition. North American Industrial Classification System (NAICS): The associated NAICS code for this procurement is 541512, with a small business size standard of $34.0M. Submission of Quotes: Offerors must follow the requirements of FAR 52.212-1 Addendum, 52.212-2 Addendum, and the Quote Instructions and Evaluation Criteria provided in the solicitation document to create the quote. Quote is required to be submitted electronically to the Contracting Officer and Contract Specialist on or before the date and time specified in the RFQ. The firewall utilized by the Government contracting office does not authorize or accept zip files. Files shall not contain classified data. The use of hyperlinks in quotes is prohibited.
