Special Notice Special Notice Page 3 of 3 Special Notice *= Required Field Special Notice Page 1 of 3 The Purpose of this Notification is to announce the Government s Intent to Exercise Contract Option 2, 3 and 4of Contract Number 36C25724P0155. This procurement was processed IAW FAR 13.5. Contracting Activity: Department of Veterans Affairs, Network Contracting Office 17, located at 5441 Babcock Road, San Antonio, TX 78240. The Contractor shall provide all materials, tools, labor, travel, and equipment necessary to provide preventive and corrective maintenance for the Cardiac Ventricular Abiomed Impellas located at the Audie L. Murphy Medical Center, San Antonio, TX. FAR 13.5 Simplified Procedures for Certain Commercial Items. The statutory authority for applying the Simplified Procedures for Commercial Items of FAR 13.5 is 41 U.S.C. § 1901 and is implemented by FAR 13.106-1(b)(2) for restricting competition on this procurement. Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements. AMBIOMED INC is the only vendor that can provide these services.
