This amendment, SPE2D222R0118 0001 1.This Amendment is issued to allow for Blister Packaging and/or Bottles to be offered in response to this solicitation. SEE THE ATTACHED REVISED SCHEDULE OF ITEMS/PRICING SHEET FOR DETAILS 2.This Amendment is herein removes and replaces the following packaging requirements as shown on solicitation pages 26, paragraph 16. Preparation for Delivery to accept either cylindrical or rectangular packaging. The following applies: REMOVE: All unit of use packaging (identified in schedule of supplies beginning on Page (2) must contain a child proof closure (safety cap) and a bottle with a minimum required size of 100cc volume. The safety cap must not exceed the diameter of the bottle. The bottle must have a cylindrical body and minimum of 5.75� circumference. The bottle must fit a label with dimensions of 5.25� x 2�. The 0.5� additional space is required to keep the bar code exposed and/or allow for the display of the expiration date and lot number. Sample pictures (bar code exposed) of how the label must fit the offered bottle are below: REPLACE with: All unit of use packaging (identified in schedule of supplies beginning on Page (2) must contain a child proof closure (safety cap) and a bottle with a minimum required size of 50cc volume. The safety cap must not exceed the diameter of the bottle. The bottle must have a cylindrical or rectangular body and minimum of 3.75� circumference. The bottle must fit a label with dimensions of 3.25� x 1.80�. The 0.5� additional space is required to keep the bar code exposed and/or allow for the display of the expiration date and lot number. Sample pictures (bar code exposed) of how the label must fit the offered bottle are below: 3.Signature is required in Block 15b as acknowledgement of amendment receipt. Signed copies are to be included with submitted proposals 4.Amendment 0001 extends the closing date to 08/26/2022 at 3:00 PM EST. 5.All other provisions, terms and conditions of the solicitation remain unchanged. NOTE: This solicitation number, SPE2D2-22-R-0118 replace synopsis/solicitation number for SPE2D2-22-R-0009 for Rizatriptan Benzoate ( SPE2D222R0009 )
