Notice of Intent to Sole Source HT941026N0002 THIS IS A NOTICE OF INTENT TO AWARD A SOLE SOURCE PURCHASE ORDER CONTRACT AND IS NOT A REQUEST FOR COMPETITIVE QUOTES. The Defense Health Agency Contracting Activity (DHACA), Healthcare Contracting Division - West (HCD-W), Alpha Branch intends to negotiate and award a firm fixed price purchase order (PO) contract for services include blood product supplies and immunohematology reference laboratory services at Nellis Airforce Base in accordance with Federal Acquisition Regulation (FAR) 13.501(a) Sole Source acquisitions and FAR 13.106-1(b)(1)(i), only one source being available to meet the Government�s needs to: Vitalant 9305 E Via De Ventura, Scottsdale, Arizona 85258-3597, UNITED STATES UEI: GKT4L2JZJ622 CAGE: 42091 The North American Industry Classification System (NAICS) code for this requirement is � 621991 Blood and Organ Banks the business size standard is $40M (Dollar amount). The Product Service Code (PSC) is - Q999 Medical - Other. This requirement is for a blood supplier and services contract for Mike O�Callaghan Military Medical Center (MOMMC) and outlines the procedures and conditions under which the blood supplier will: (a) distribute blood/blood components to the Installation, and (b) provide immunohematology reference laboratory services to the installation. Location: 5865 SWAAB BLVD, NELLIS AFB, NV 89191-7063 United States Period of Performance (PoP): BASE: 01NOV25 TO 31OCT26 OPTION 1: 01NOV26 TO 31OCT27 OPTION 2: 01NOV27 TO 31OCT28 OPTION 3: 01NOV28 TO 31OCT29 OPTION 4: 01NOV29 TO 31OCT30 Based on market research and previous acquisition history, it has been determined that Vitalant is the sole provider of blood supplies and services capable meeting the needs of the SOW. This notice of intent is not a request for competitive proposals, and no solicitation document exists for this requirement. Sources interested in responding to this notice are required to submit a capability statement that includes management and technical data and cost information, in sufficient detail and with convincing evidence that clearly demonstrates the capability to provide the required cornea tissue locally. Capability statements shall not exceed six (8.5 x 11 inch) pages using a font size no smaller than 10-point. All capability statements received by the due date of this notice will be considered by the Government. A request for documentation or additional information or submissions that only ask questions will not be considered as an affirmative response. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement or to proceed with a sole source purchase order.
