Description

This is a combined synopsis/solicitation for items prepared in accordance with FAR Part 13. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) 246-26-Q-0007 This procurement is 100% Buy Indian owned set-aside under NAICS code 238220 with a size standard in millions of dollars $19.0. The Contractor shall remove, replace, and install a corridor door and pass-through specimen window for the Laboratory at Haskell Indian Health Center in Lawrence, Kansas as shown on the drawing and the statement of work. The period of performance for this requirement is 120 calendar days from Notice to Proceed. Questions are due no later than January 16, 2026 5:00 p.m. CST. The closing date for receipt of quotes is February 02, 2026 COB CST. The offer shall be received by email to Noleen.Powell@ihs.gov or in a sealed envelope, addressed to this office, showing the solicitation number, your name and address, no later than 5:00 p.m. CST, on February 02, 2026. CLAUSES INCORPORATED BY FULL TEXT: Please note that due to character limitation in SAM.gov, full text of the Federal Acquisition Regulation (FAR) and Health and Human Services Acquisition Regulations (HHSAR) can be accessed on the Internet at http://www.acquisition.gov and acquisition.gov/hhsar FAR 52.213-4 Terms and Conditions-Simplified Acquisitions (Other Than Commercial Items)(OCT 2025) The estimated price between $25,000 and $100,000. THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO PROVIDE A PROPOSAL CONTAINING: A) Firm, Fixed price quote on the SF18 form and a completed attached bid sheet. B) Technical specifications of the statement of work, acceptable materials list to be used. C) (2) Relevant past performance of this type of work, shall provide organization�s name, point of contact, summary of project and award amount. D) Bid bond form (SF24) attached is required at $35,000. If over $35,000 and selected as the acceptable vendor proposal, performance (SF25) and payment (SF25A) bonds shall apply. Award amount is estimated to between $25,000 and $100,000. Offers shall be evaluated on an acceptable/non-acceptable basis. Unacceptability of "A" and "B" (stated above) shall render the entire quote unacceptable. Evaluation shall proceed as follows: 1. Quotes shall be ranked by price, from low to high 2. Quotes shall then be evaluated for technical acceptability beginning with the lowest priced quote 3. This process shall proceed until a technical acceptable quote is identified THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO MEET THE REQUIREMENTS IN THE STATEMENT OF WORK CONTRACT AWARD SHALL BE MADE TO THE RESPONSIBLE, RESPONSIVE OFFEROR OFFERING THE LOWEST REASONABLE PRICE AND MEETING ALL TECHNICAL SPECIFICATIONS. The technical evaluation shall be a determination based on information furnished by the vendor in the submitted proposals. The Government is not responsible for locating or securing any information which is not identified in the offer. The Government reserves the right to make an award without discussions.
