***Amendment 0001 is incorporated into the solicitation. See attached PDF with submitted questions/answers regarding the solicitation. Also, Amendment 0001 is removing 52.219-6 Notice of Total Small Business Set-Aside, which was included by mistake. See Rev. 1 of the solicitation, which has removed this clause. The due date for quotes is not being extended and remains the originally specified date/time. Please sign and return a copy of this amendment with all submitted quotes.*** DLA Distribution Oklahoma City, Oklahoma has a requirement for an estimated (100) pre-employment physicals to be conducted over the next year. Please direct any questions and email all quotes to John.Sproul@dla.mil. Due date for quotes is 21 November 2025 at 1:00PM EST. 1-Taxes do not apply as DLA is a Federal Agency and is tax exempt. 2- Payment will be made by WAWF. 3-Contractor's facility must be located within a 30-mile one way radius of DLA Distribution Oklahoma City. 4-Contractor must also hold a current, unrestricted medical license in the State of Oklahoma.
