***Amendment 0001 is hereby issued to add "Q_A - 75H70726Q00054" to attachments and to extend response date to Tuesday, June 16, 2026 by 5:30pm MDT. To replace "Brand Name Justification" with redacted version*** (i) This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with part 12. This announcement constitutes the only solicitation. Quotes are being requested and a separate written solicitation will not be issued. Solicitation No. 75H70726Q00054 is issued as a Request for Quotation (RFQ) for medical equipment replacement/upgrade for the Nursing Department located at the Ute Mountain Ute Health Center in Towaoc, CO. This facility is part of the Albuquerque Area Indian Health Service (IHS), an Agency under the Department of Health & Human Services (HHS). This acquisition is set-aside for Indian Small Business Economic Enterprise (ISBEE) concerns. The full text of provisions and clauses may be accessed electronically at www.acquisition.gov. (viii) QUOTATION SUBMISSION REQUIREMENTS To be considered for award, vendors shall submit: Pricing Completed pricing schedule or equivalent quote identifying unit prices, extended prices, delivery charges (if applicable), and total quoted price. Technical Information Documentation demonstrating the ability to provide all requested equipment in accordance with the specifications and brand name requirements identified in this solicitation. Product literature, specifications, or manufacturer documentation may be submitted to demonstrate compliance. Indian Economic Enterprise Representation A completed and signed representation meeting the requirements of HHSAR 352.226-7, Indian Economic Enterprise Representation. SAM Registration Offerors must be actively registered in SAM.gov at the time of quotation submission and award. Required Representations Completed representations required by this solicitation, including FAR 52.209-2 and FAR 52.209-11, if not already completed within the quotation package. Past Performance Information Provide up to three recent contracts or purchase orders for similar supplies performed within the last three years, including customer name, point of contact, phone number, and brief description of supplies provided.
