Description
The VA Heartland Health Care Network (VISN 15) requires a VISN-wide contingency IDIQ Requirements contract for all medications, diluents, fluids, IV Tubing, closed system drug transfer devices and any other compounding and administration supplies items deemed necessary for the provision of hazardous and non-hazardous compounded intravenous products. (Attachment D. 1) The quantities reflected in Attachment D.1 is estimated annual requirements provided for pricing and planning purposes. These figures do not represent guaranteed purchase commitments. Actual quantities ordered under this VISN-wide contingency Requirements contract will vary based on patient-specific needs and operational requirements throughout the contract term. The government reserves the right to order more or fewer quantities than estimated. All sterile products will be aseptically compounded based on patient specific orders, in accordance with federal, state and local laws and any regulatory or accrediting bodies. Additionally, the vendor shall provide all deliveries and handling of materials and supplies to ensure appropriate storage conditions are maintained during shipment, to include segregated shipment and labeling for hazardous versus non-hazardous sterile compounds. Pricing in B.2 Price/Cost Schedule is all inclusive. Contractor shall provide and update the �VHA Pharmacy Benefits Management (PBM) Assessment Guide for Compounded Sterile Preparations within VAMCs and Outsourced Compounding Pharmacies� at time of solicitation response and annually thereafter (See Attachment D. 2 VHA PBM)
