The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI 53214, is performing market research to determine if there is a sufficient number of Authorized and qualified: (1) Service Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); or (3) Small or emerging small business firms, or (4) Other Than Small business firms, to provide Carl Zeiss Visulas Combi YAG Green Laser at the Capt. James A Lovel VA Medical Center. General Requirements: STATEMENT OF WORK (SOW) Project Title: Installation, Setup, and Commissioning of Carl Zeiss Visulas Combi YAG/Green Laser System Facility: Captain James A. Lovell Federal Health Care Center (FHCC), North Chicago, IL Prepared For: Summers, Rodney D., Health Tech 1. Purpose The purpose of this Statement of Work is to define the requirements, deliverables, and responsibilities associated with the procurement, installation, configuration, and validation of the Carl Zeiss Visulas Combi YAG/Green ophthalmic laser system. 2. Scope of Work Delivery of the Carl Zeiss Visulas Combi YAG/Green unit and accessories Installation, testing, and calibration Configuration and verification of system performance Safety and functional checks Staff training 3. Equipment Specifications Carl Zeiss Visulas Combi YAG/Green laser Footswitch, slit-lamp integration, eyewear, manuals 4. Installation Requirements Room suitability verification Beam alignment and calibration Safety interlock confirmation 5. Training Operator training Maintenance overview Laser safety overview 6. Deliverables Installed and tested system Manuals and documentation Warranty information Training certification 7. Warranty and Support Minimum 12-month warranty Service response requirements 8. Compliance and Safety FDA guidelines compliance ANSI Z136.1 laser safety standards VA/DoD facility requirements 9. Responsibilities Vendor: Delivery, installation, training, documentation FHCC: Provide access, room readiness, staff availability 10. Acceptance Criteria System accepted when installation, calibration, and training are complete. 11. Period of Performance Delivery and installation timeline TBD Training within X days of installation VAAR 852.212-71 GRAY MARKET ITEMS will be applicable to any request for quote for this item. All interested authorized distributor firms who can meet the requirements stated above should respond, in writing, INCLUDING ALL THE FOLLOWING: Company name. Address. Indication of which business category (SDVOSB, VOSB, Small Business or Other Than Small Business) for which the organization qualifies. A capability statement that addresses the organizations qualifications and ability to provide the requirement depicted above. Documentation from manufacturer (i.e. correspondence from manufacturer) of your firm being an authorized distributor for the manufacturer. TELEPHONIC INQUIRIES WILL NOT BE ACCEPTED. The NAICS Code 339115 Ophthalmic Goods Manufacturing, is applicable to this acquisition; the size standard is 1,000 employees. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Systems for Award Management (SAM) database prior to award of a contract. The Government will use responses to this notice to make an appropriate acquisition decision. This is the only notice; no other copies of this notice will be provided.
