36C25026Q0608 SPECIAL NOTICE NOTICE OF INTENT TO SOLE SOURCE Phacoemulsification Device, COLUMBUS The Department of Veterans Affairs, The Chalmers P. Wylie VA Ambulatory Care Center, Columbus OH, 420 N. James Road, Columbus, Ohio 43219 intends to solicit and negotiate with only one source under the authority of FAR 6.302-1(a), Only one responsible source and no other supplies or services will satisfy agency requirements. The VA intends to solicit and negotiate with O.R. Elder Inc. the sole authorized Service Disabled Veteran Owned Small Business (SDVOSB) Distributor, 1270 Broadway, Suite 401, New York, NY 10001 for ALCON. The Surgery Services at the Chalmers P. Wylie VA Ambulatory Care Center is interested in purchasing 2 replacement phacoemulsification devices and 17 4D handpieces. These items are used daily in our surgery department for cataract extraction and lens implant. The number of handpieces are needed for daily reprocessing and availability for cataract surgeries. The phacoemulsification device has a graphical operator interface that is menu driven. The Government believes that only O.R. Elder Inc. can satisfy the agency requirements due to our facility currently utilizing Alcon cataract disposables, that are used with current compatible devices, with Unity CS CTP for use during phacoemulsification and removal of cataracts. We intend to negotiate with only one source under the authority of FAR 6.302-1(a)(2), Only one responsible source and no other supplies or services will satisfy agency requirements. Other persons may identify their interest and capability to respond to this requirement by Wednesday, June 3, 2026 by 5PM EST. Such capabilities information will be used solely for the purpose of determining whether or not to conduct this procurement on a competitive basis.� All information shall be furnished at no cost or obligation to the Government.� A determination by the Government not to open the requirement to competition based upon responses to this notice is solely within the discretion of the Government. DEPARTMENT OF VETERANS AFFAIRS Justification for Single Source Awards IAW FAR 13.106-1 For Over Micro-Purchase Threshold but Not Exceeding the Simplified Acquisition Threshold Acquisition Plan Action ID: 36C250-26-AP-3006 Contracting Activity: The Network Contracting Office (NCO) 10, 2780 Airport Drive, Columbus, Ohio 43219 for the Department of Veterans Affairs, VISN 10, Chalmers P. Wylie VA Ambulatory Care Center, 420 North James Rd, Columbus, Ohio 43219 using transaction # 757-26-2-096-0239. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The products proposed for purchase are upgraded end of life Phacoemulsification consoles and upgraded Ozil Handpieces. Unity CS CTP is a multifunctional surgical cataract system indicated for use in anterior segment ophthalmic surgeries. Used to facilitate management of fluid as well as removal, cutting, and coagulation of ocular materials. Alcon 4D phacoemulsification Handpieces deliver up to 2x faster nucleus removal than Ozil and 41% less energy into the eye. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: Alcon Unity CS CTP feature many first-to-market technologies designed to deliver transformative surgical innovation for cataract surgery. Unity CS is designed to allow cataract surgeons to operate with increased stability and efficiency at more physiologic (normal) conditions through its Intelligent Fluidics functionality. This new technology maintains a consistent IOP (Intraocular Pressure) level to help support patient comfort during surgery, while allowing for higher average vacuum during lens removal. Additionally, the system offers two surgical performance enhancements: the Unity Intelligent Sentry real time occlusion break sensing and dual venting that is better at maintaining anterior chamber stability. And Thermal Sentry is the first phacoemulsification handpiece that features a thermal sensor that works with a proprietary algorithm to estimate incision temperature in real-time and regulate ultrasound power. 4D phacoemulsification handpieces integrate irrigation, aspiration, and emulsification to simultaneously maintain or inflate the anterior chamber, emulsify the lens, and aspirate the lens material from the eye. This is 2x faster nucleus removal than an Ozil with 40% less energy into the eye. This advanced technology gives the Veteran less discomfort during the procedure, shorter operating time with better outcomes Description of market research conducted and results or statement why it was not conducted: Market research not conducted by the End User due to our facility currently utilizing Alcon cataract disposables, that are used with current compatible devices, with Unity CS CTP for use� during phacoemulsification and removal of cataracts. Market research was conducted by the Contracting Officer to locate adequate concerns on GDA, FPDS, SAM, SBA and the Internet. Due to compatibility Alcon or their sole authorized distributor are the only Vendors who can satisfy this requirement.
