AGENCY Department of Health and Human Services (HHS) Indian Health Service (IHS) Great Plains Area Indian Health Service Pine Ridge Service Unit TITLE EMS Cot and Associated Accessories for Pine Ridge Service Unit NAICS CODE 339113 � Surgical Appliance and Supplies Manufacturing Size Standard: 800 Employees DESCRIPTION The Indian Health Service (IHS), Pine Ridge Service Unit (PRSU), is conducting market research to identify qualified Indian Economic Enterprises (IEEs), Indian Small Business Economic Enterprises (ISBEEs), and other small business concerns capable of providing an ambulance cot system and associated accessories that meet the Government's requirements. The Government anticipates a Firm-Fixed-Price purchase order for the acquisition of one (1) ambulance cot system and associated accessories for use in a patient transport vehicle equipped with a Ferno Stattrac loading system. The requirement is for a Ferno 35-XST Proflexx Ambulance Cot system or equal. Any proposed equal product must meet or exceed the Government's minimum requirements and salient characteristics and be fully compatible with the existing Ferno Stattrac loading and retention system. Minimum salient characteristics include: � Manual ambulance cot designed for emergency medical services patient transport. � Compatible with existing Ferno Stattrac loading and retention systems without modification to Government-owned equipment. � Capable of accommodating adult patients in accordance with manufacturer specifications. � Adjustable patient transport surface and side-arm configuration. � Includes mattress and integrated patient restraint system. � Durable construction suitable for EMS operations. � Manufacturer warranty and availability of replacement parts and service. � Safe loading, unloading, and transport capability in a healthcare transport environment. Required equipment includes: � Ambulance cot with mattress and restraints (Ferno 35-XST Proflexx or equal) � Heavy-duty IV pole (or equal) � Equipment mounting platform (or equal) � Storage net system (or equal) � Bariatric board attachment (or equal) � Mounting hardware (or equal) � Compatible bariatric mattress (or equal) The anticipated place of delivery is: Pine Ridge Hospital 607 Indian Health Road Pine Ridge, SD 57770 The anticipated delivery period is within ninety (90) calendar days after award. REQUESTED INFORMATION Interested firms shall submit the following information: 1. Company name, address, UEI number, and point of contact. 2. Business size and socioeconomic status, including whether the firm is: � Indian Economic Enterprise (IEE) � Indian Small Business Economic Enterprise (ISBEE) � Small Business � Small Disadvantaged Business � HUBZone Small Business � Service-Disabled Veteran-Owned Small Business � Woman-Owned Small Business � Other 3. Completed Buy Indian Representation Form. 4. Confirmation that the firm is registered in SAM.gov and that registration is active. 5. Description of the product proposed, including manufacturer and model number. 6. Technical literature, specifications, brochures, and compatibility documentation demonstrating compliance with the Government's salient characteristics. 7. Description of previous experience providing similar ambulance cots, patient transport equipment, or EMS equipment. 8. Estimated delivery lead time after receipt of order. 9. Identification of whether the proposed product is the referenced brand name or an equal product. BUY INDIAN ACT INFORMATION Indian Economic Enterprises (IEEs) and Indian Small Business Economic Enterprises (ISBEEs) responding to this notice shall identify their eligibility under the Buy Indian Act, 25 U.S.C. � 47, and the implementing regulations at HHSAR Subpart 326.6. The Government will use responses to determine whether sufficient competition exists among eligible IEE/ISBEE firms to support a Buy Indian set-aside acquisition. SUBMISSION INSTRUCTIONS Responses shall be limited to five (5) pages, excluding product literature and capability documentation. Response will also include a completed Buy Indian Representation form. No contract will be awarded as a result of this notice. The Government reserves the right to determine the appropriate acquisition strategy based on the responses received. NOTICE REGARDING GOVERNMENT AUTHORITY Responses to this Sources Sought Notice shall not be construed as an obligation on the part of the Government. Only a duly warranted Contracting Officer has the authority to commit the Government to the expenditure of public funds or to otherwise bind the Government contractually. No information obtained from any other Government employee shall be considered binding on the Government.
