Description
This is a Special Notice for commercial items prepared in accordance with FAR Subpart 5.2, Synopsis of Proposed Contract Actions. This announcement constitutes the only Notice of Intent (NOI) to Sole Source (SS) and synopsis of proposed action. The Department of Veterans Affairs Michael E. DeBakey VA Medical Center (MEDVAMC) Network Contracting Office (NCO) 16 intends to issue and award a Sole Source contract to Premier Medical Distributers LLC located at 141 Chapel Ridge Circle, Myrtle Beach, SC 29588, for the installation and site prep of overhead patient lifts at the MEDVAMC, located at 2002 Holcombe Boulevard, Houston, TX 77030. It has been determined that through initial Market Research, only 1 (one) responsible SDVOSB source per FAR 19.1406 can provide the required upgrades as identified below. The statutory authority for this non-competitive acquisition as implemented by FAR 6.302-5(b)(6) Sole source awards under the Veterans Benefits Act of 2003 . The North American Industrial Classification System (NAICS) code for this procurement is 339113 Surgical and Medical Instrument Manufacturing, with a small business size standard of 800 employees, and the FSC/PSC is 6515 Surgical and Medical Instrument Manufacturing. The Department of Veterans Affairs intends to solicit and award IAW FAR 8.002 a sole source firm fixed contract for Guldmann patient lifts shown below: DESCRIPTION QTY GH3+ 605lb SWL, wifi capable, CLM and service modules, hanger bar, rails, trolleys, hardware - 3mx3m or smaller. 103633 Proctology Clinic. 1 GH3+ 605lb SWL, wifi capable, CLM and service modules, hanger bar, rails, trolleys, hardware - 3mx3m or smaller. 103203 Radiology. 5 GH3+ 605lb SWL, wifi capable, CLM and service modules, hanger bar, rails, trolleys, hardware - 3mx3m or smaller. 102371 GI Clinic. 6 GH3+ 605lb SWL,duynaic lift system, integrated scale wifi capable, CLM and service modules, hanger bar, rails, trolleys, hardware- 5mx5m or smaller. 103568 SCI Unit PT Gym. 1 GH3+ 605lb SWL, wifi capable, CLM and service modules, hanger bar, rails, trolleys, hardware - 4mx3m. Cancer Clinic. 1 GH3+ 605lb SWL, wifi capable, CLM and service modules, hanger bar, rails, trolleys, hardware - 4mx3m. 5H Surgery Center 1 GH3+ 605lb SWL, wifi capable, CLM and service modules, hanger bar, rails, trolleys, hardware - 4mx3m. Dental Clinic. 4 GULDMANN CERTIFIED INSTALLATION. Non Union Labor [and hardware] to install lifts in a typical 4m x 3m room. 1 room per day, [2] mobilization(s) of installation team. Electrical work, existing structure demo, modification, or reinstallation including but not limited to ceilings, walls, tile, patching, painting, acoustic caulking, asbestos abatement, smoke/fire wall repair, fireproofing reapplication, sprinkler relocation and door header modification is outside the scope of work for Guldmann installers and Guldmann is not licensed for performing such work, nor is Guldmann responsible for hiring specific contractors/trades to perform such work. ICRA up to level II provided; additional is responsibility of facility. Guldmann will perform light cleaning of the area but returning room to original condition is outside of scope of work for Guldmann installers. If an Above Ceiling Work Permit is required, facility must provide. 18 GULDMANN CERTIFIED INSTALLATION. THIS NOTICE IS NOT A REQUEST FOR COMPETITIVE PROPOSALS OR QUOTATIONS. A SOLICITATION WILL NOT BE POSTED. Telephone requests will not be honored. No award will be made based on quotes or offers received in response to this notice. All responsible sources may identify their interest to meet this requirement to Robert Arabie by response date published on page 1.
