Description
REQUEST FOR INFORMATION (RFI) Custom Orthotic Insoles Blanks, Cork Components, Plastazote/Poron Materials, and Toppings Tomah VA Medical Center (Tomah VAMC) NAICS: 339113 Surgical Appliance and Supplies Manufacturing PSC: 6515 Medical and Surgical Instruments, Equipment and Supplies 1. PURPOSE The Department of Veterans Affairs (VA), Tomah VA Medical Center, located at 500 E. Veterans Street, Tomah, WI 54660 is conducting market research to identify qualified vendors capable of supplying orthotic blanks, cork components, Plastazote/Poron laminates, top-cover materials, and die cut insole components required for the fabrication of custom-fit insoles. This RFI is issued solely for information and planning purposes and does not constitute a solicitation. The VA seeks information from all capable sources, including VOSB/SDVOSB, Small Business, and other business classifications. Vendors currently holding VA FSS 65 II A or GSA MAS contracts are strongly encouraged to respond. 2. REQUIREMENT DESCRIPTION Tomah VAMC requires the following items to replenish inventory used in the fabrication of custom orthotic shoe inserts. Vendors must indicate their ability to supply all or part of the items listed. C. Technical Capability Ability to supply each of the required items Manufacturer information (if different from vendor) and country of origin Product sheets or equivalent documentation for blanks, cork, Plastazote/Poron, and toppings Lead times and shipping details D.
